RACI matrices encode decision rights: Accountable identifies the single party finally answerable for outcomes on a critical data element or system. Leaving Accountable blank creates orphaned ownership and stalled escalation. Responsible execution, or Consulted/Informed roles, cannot replace that accountability. Governance design reviews should reject incomplete RACIs for critical assets before go-live of stewardship models.
